CagriSema
Multi peptide
ABOUT
CagriSema is a fixed-dose combination of cagrilintide 2.4 mg and semaglutide 2.4 mg given once weekly. Unlike the other blends in this category it has been through phase 3 and published in the New England Journal of Medicine; it is not yet approved.

IDENTITY
HALF-LIFE
HOW IT IS THOUGHT TO WORK
Two complementary satiety pathways rather than one reinforced. Semaglutide is a GLP-1 receptor agonist acting on glucose-dependent insulin secretion, glucagon suppression, gastric emptying and central appetite signalling; cagrilintide is a long-acting amylin analogue acting on calcitonin and amylin receptors in the hindbrain to promote satiation. Amylin and GLP-1 are distinct hormones with distinct receptors, which is why the combination was developed rather than a higher dose of either.
PHARMACOKINETICS
Both components were engineered for once-weekly subcutaneous dosing - semaglutide by fatty-diacid albumin binding and DPP-4-resistant substitution, cagrilintide by acylation - so unlike the vendor blends the two halves of this vial are cleared on deliberately matched timescales. A phase 1b study characterised the two given together before the phase 3 programme began.

PUBLISHED PAIR RECORDS
with Cagrilintide
Same substance taken twice: blend already contains this compoundCagriSema already contains Cagrilintide at 2.4 mg per vial. Scheduling the standalone compound alongside the blend means taking it twice from two sources, and the blend's other components come with it whether or not they are wanted at that moment. This is the one exposure risk a blend creates that its individual parts do not, and it is arithmetic rather than pharmacology - the combined amount is whatever the two products add up to. Where a blend's ratio is fixed by the manufacturer, the only way to change one component's dose is to add or remove the whole vial. Peppy reports curated pharmacology, not a safety verdict. A prescriber or pharmacist should review any intended combination.

Overlapping blends: the same compounds appear in bothKyinsu and CagriSema both contain Semaglutide. Taking both means those shared components arrive from two vials at once, at a combined amount neither product states, while the components unique to each still arrive at their own. Because both are sold at fixed manufacturer ratios, neither can be adjusted to compensate for the other. Peppy reports curated pharmacology, not a safety verdict. A prescriber or pharmacist should review any intended combination.

Taking the blend therefore carries the same consideration as taking that component on its own, with the difference that a fixed-ratio vial cannot be adjusted independently of the rest of its contents. Reviewed 25 Aug 2026HUMAN EVIDENCE
REDEFINE 1 was a 68-week double-blind phase 3 trial in 3,417 adults with obesity or overweight and at least one comorbidity, without type 2 diabetes, randomised to CagriSema, semaglutide alone, cagrilintide alone or placebo. Mean weight change was -20.4% against -3.0% for placebo on the treatment-policy estimand, and 22.7% on the trial-product estimand; semaglutide alone reached 16.1% and cagrilintide alone 11.8%, so the combination beat both monocomponents. REDEFINE 2 covered adults with type 2 diabetes. An important qualifier sits in the dosing: only 57.3% of CagriSema participants were on the highest dose at 68 weeks, against 82.5% for cagrilintide and 70.2% for semaglutide, and the trial fell short of the 25% figure the sponsor had signalled.
REPORTED EFFECTS
The most common adverse events were gastrointestinal, mostly mild to moderate and diminishing over time, consistent with the GLP-1 class. A REDEFINE 1 analysis published in Hypertension reported reduced blood pressure. There is no approved label yet, so there is no regulator-verified adverse-reaction table - the safety picture is trial-reported rather than label-verified, and the class warnings that apply to semaglutide have not yet been formally extended to this combination.
RESEARCH LIMITATIONS
This is a summary of a blend, not a recommendation. Its composition is given as commonly marketed and is deliberately uncited: the only places it is stated are the shops selling it, which this catalogue does not link and does not treat as evidence. Treat the amounts as unverified, and note that blends sold under one name are not standardised between suppliers. Read the mechanism, pharmacokinetic, human-evidence and reported-effect fields separately: a mechanism is not an outcome, and a combination of mechanisms is not a combination of outcomes.
